本合同包不接受联合体投标

3.本项目的特定资格要求：

合同包1(产品开发反应器)特定资格要求如下:

1）投标人须为具有独立承担民事责任能力的法人、事业法人、其他组织或自然人。企业法人应提供有效存续的企业营业执照及2021年度年检报告；事业法人应提供事业单位法人证书；其他组织应提供合法登记证明文件；自然人应提供身份证；？2）投标人须为具备该项目产品供货 (略) 家或经销商， (略) 家参与投标的需提供具有本项目产品制造能力的证明资料，经销商参与投标的需提供 (略) 家给定的针对该项目产品的代理授权书证明资料（包括但不限于产品制造商授权、销售协议、代理协议等）；3）财务状况报告：提供2021年度完整的财务审计报告（成立时间至提交响应文件截止时间不足一年的可提供成立后任意时段的现金流量表、资产负债表和利润表或其基本存款账户开户银行出具的资信证明及基本存款账户开户许可证）；4）税收缴纳证明：提供2021年01月至今已缴纳的至少一个月的纳税证明（银行缴费凭证）或完税证明，依法免税的单位应提供相关证明材料；5）社会保障资金缴纳证明：提供2021年01月至今已缴纳的至少一个月的纳税证明或完税证明，依法免税的单位应提供相关证明材料；？6）参加本次政府采购活动前3年内在经营活动中没有重大违纪，以及未被列入失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信行为记录名单的书面声明；7）提供具 (略) 必需的设备和专业技术能力的证明资料或承诺书；8）供应商不得为“信用”网站（www.credi 点击查看>> ）中列入严重失信主体名单和税收违法黑名单的当事人名单企业，不得为企业信用信息公示系统（http:/ 点击查看>> ）列入经营异常名录信息、列入严重违法失信名单（黑名单）信息，不得为政府采购网（www.ccg 点击查看>> ）政府采购严重违法失信行为记录名 (略) 门禁止参加政府采购活动的企业（提供网页查询截图和“信用”、“企业信用信息公示系统”网站下载的企业信用报告并加盖企业原色印章，信用报告下载时间须从招标文件发出开始至投标截止时间内）；？9）提供榆林市政府采购货物类项目供应商信用承诺书原件（格式详见招标文件）；10）供应商应在“信用（陕西榆林）”网站进行注册、登录、并提供截图，自主上报信用承诺书（网址：https:/ 点击查看>> ）；11）投标保证金缴纳凭证。备注：1）本项目不接受联合体投标，不允许分包。投标人提供《非联合体、不分包投标声明》，视为独立投标，不分包。2）单位负责人为同一人或者存在直接控股、管理关系的不同投标人，不得参加同一合同项下的政府采购活动；（3）事业单位法人参与投标可不提供财务 (略) 会保障资金缴纳证明及税收缴纳证明。
