资格要求
1.应答人具有合法有效的企业法人营业执照、税务登记证及组织机构代码证或证照合一的营业执照，应答时需提供原件扫描件（原件备查）。应答人为事业单位的，应具有合法有效的事业单位法人证书，应答时需提供原件扫描件（原件备查）。 (略) 的，应具有合法有效的营业执照和上级法人单位授权书，分公司与上级法人单位只可一家参与应答，同时参与应答的，应答均无效。中国境外注册的应答人应具有符合注册地要求的营业执照。The respondent must have a valid and legal business license, tax registration certificate, organization code certificate, or a business license that combines certificates and licenses. Respondents registered outside of China shall have a business license that meets the requirements of the place of registration.2.业绩要求：应答人自2021年1月1日起至应答截止时间止，完成不少于1个合同含税金额为20,000元及以上海外补充医疗保险服务类合同业绩。原合同签订时间早于2021年1月1日，续签时间在2021年1月1日起至应答截止时间的合同视为无效业绩。业绩证明材料包括但不限于：1）合同扫描件（含相关技术附件，原件备查）和2）服务验收材料。证明文件必须至少体现以下内容：合同签署时间（自2021年1月1日起至应答截止时间）、盖章页、合同金额（合同含税金额为20,000元及以上）、项目名称、项目内容（海外补充医疗保险服务类）、服务验收材料（合同*方签字或盖章的验收证明，或20,000元及以上付款结算发票）。未提交业绩证明文件，或所提供的业绩证明文件未体现任意一项的，均视为无效业绩。Performance requirements: The respondent shall complete no less than one Overseas Supplementary Medical Insurance service related contract with total price including tax RMB20,000 or more performance from January 1, 2021 to the response deadline. Original Contracts signed earlier than January 1, 2021 and renewed between January 1, 2021 and the deadline for response shall be deemed invalid performance.The performance certification materials include but are not limited to: 1) scanned copies of the contract (including relevant technical attachments, original copies for reference) and 2) service acceptance materials. The supporting documents must at least reflect the following contents: contract signing time (from January 1, 2021 to the deadline for response), stamped page, contract amount (contract tax inclusive amount of * yuan or more), project name, project content (Overseas Supplementary Medical Insurance services), service acceptance materials (acceptance certificate signed or stamped by Party A of the contract, or * yuan or more payment settlement invoice).
