五、投标人资格要求：

1、本次招标要求投标申请人必须具有独立承担民事责任能力，遵守国家法律法规的具有独立法人资格的法人及其他组织；

3、本项目针对拟采购货物，同一品牌只接受一家货物制造商或代理商参与本次投标活动（以投标登记的先后顺序为准），如为代理商需提供货物制造商针对本项目出具的授权委托书或产品经销协议；

六、招标文件获取方式及时间：

6.1潜在供应商请于2020年7月9日至2020年7月16日（双休日除外），每日9时至16时（北京时间，休息时间除外）在中资国际工程咨询集团有限责任公司（哈尔滨市南岗区汉水路76-6）进行获取招标文件，逾期不予受理，文件售价￥500/$73/包，售后不退(国内邮购须另加100元人民币；国外邮购须另加20美元)，售后不退。

6.1 Potential suppliers are requested to obtain bidding documents from China International Engineering Consulting Group Co., Ltd. (76-6 Hanshui Road, Nangang District, Harbin) from 9:00 to 16:00 every day (Beijing time, except rest time) from July 9, 2020 to July 16, 2020 (except weekend). If the bidding documents are overdue, the price of the documents is ￥ 500 / $73 / package, and the after-sales service is non refundable (additional 100 yuan for domestic mail order) RMB; foreign mail order must add 20 US dollars)..

6.2、获取文件时需提供以下材料：

1）法人授权委托书（法人身份证复印件加盖公章）；

2）委托代理人身份证（原件）；

3）营业执照；

4）基本账户开户许可证或基本存款账户信息单；

5）“中国政府采购网”、“信用中国”、“国家企业信用信息公示系统”、“中国执行信息公开网-全国法院失信被执行人名单信息公布与查询”平台的信用查询截图；

6）代理商需提供货物制造商针对本项目出具的授权委托书或产品经销协议；

7)参加本项目投标的潜在供应商须具备备医疗器械生产许可证（生产商）或医疗器械经营许可证（经销商）；

获取文件时，上述材料均须提供原件以及原件的复印件（加盖投标人公章），复印件须清晰、完整且加盖企业公章，潜在投标人应对所提供材料的合法性、真实性、完整性负责，资料不全，将不予受理。招标人将对投标人提供的资料进行核实，一旦发现有弄虚作假的情况，投标人登记无效。

6.2. Bidders are requested to fill in the “Registration Form for Bidding and Purchase of Bidding and Purchasing Projects of Heilongjiang Mudanjiang Forestry Center Hospital” attached to this announcement. The following materials are required for registration:

1) Power of attorney of legal person (copy of legal person identity card with official seal);

2）The identity card of the principal agent (original);

3) Business license;

4) The basic account opening license or Basic deposit account information sheet;

5) Screenshots of credit queries on the platforms of "China government procurement network", "credit China", "national enterprise credit information publicity system", "China executive information disclosure network - information release and inquiry of the list of people subjected to execution for breach of trust by courts across the country";

6) The agent shall provide the power of attorney or product distribution agreement issued by the manufacturer for the project;

7)Potential suppliers participating in the bidding of this project shall have a medical device manufacturing license (manufacturer) or a medical device trading license (distributor).

At the time of registration, the above materials shall be provided with the original and the color printed copy of the original (stamped with the official seal of the bidder). The color printed copy shall be clear, complete and stamped with the official seal of the enterprise. The potential bidder shall be responsible for the legality, authenticity and integrity of the materials provided. The tenderer shall verify the information provided by the bidder, and the registration of the bidder shall be invalid once fraud is found.

6.3在受理投标登记后，拟派的项目经理、技术负责人及授权委托代理人在参加本项目投标全过程中不得随意更换（不可抗力的情况除外）。
