3、投标人资格要求
投标人应具备的资格或业绩:1)如果投标人按照合同提供的货物不是投标人自己制造的，投标人应得到货物制造商同意其在本次投标中提供该货物的正式授权书原件或制造商出具的长期代理经销协议（复印件）；
且：a. 如果授权是二级或二级以下的，必须提供上一级别的授权，并且制造商对所作的所有承诺负完全责任。
b.如果授权书中明确规定不能转授权的，转授权无效，视为未提供授权书。
c.不接受制造商的分公司出具的授权书。
2)证明投标人已具备履行合同所需的财务、技术和生产能力的文件，包括：
a．投标设备必须取得产品的《中华人民共和国医疗器械注册证》（含《医疗器械产品注册登记表》及其它相关附件）（根据中华人民共和国国务院令2014年第650号《医疗器械监督管理条例》和国家药品监督管理局关于印发《医疗器械分类目录》的通知（国药监械【2002】302号）规定及国家药品监督管理局发布的该目录的修订，在《医疗器械分类目录》内的产品必须按照《医疗器械监督管理条例》的要求具备相关资质，并提供真实的证明材料，其他不在《医疗器械分类目录》内的设备不作强行要求）。
b．如果投标人工商注册地在中华人民共和国境内的，须提供具备所投设备经营范围的《中华人民共和国医疗器械经营企业许可证》或《中华人民共和国医疗器械生产企业许可证》，以许可证上的经营范围为准，不接受其他证明材料。
3)投标人开户银行在开标日前三个月内开具的资信证明或资金存款证明（原件或该原件的复印件）；
4）由检察机关开具的投标人企业及法定代表人无行贿犯罪查询记录。
