六、供应商资质
1具有企(事)业法人资格(有行业特殊的银行、保险、电力、电信等法人分支机构,会计师、律师等非法人组织,行业协会等社会团体法人除外)。
2.国有企业;事业单位;军队单位;成立三年以上的非外资(含港澳台)独资或控股企业,国内市场无类似或可替代产品的企业除外。
3.具有良好的商业信誉和健全的财务制度。
4.有依法缴纳税收和社会保障资金的良好记录。
5.参加军队采购活动前3年内,在经营活动中没有受到刑事处罚或者责令停产停业、吊销营业执照或者执照、200万以上罚款等重大违法行为。
6.未被列入政府采购失信名单、军队采购暂停名单,未在军队采购失信名单禁入处罚期内,未被政府主管部门列入违法失信名单、重大税收违法案件当事人。
7.单位负责人为同一人或者存在直接控股、管理关系的不同供应商,不得同时参加同一包的采购活动。生产型企业的生产场经营地址或者注册登记地址为同一地址的,非国有销售型企业的股东和管理人员(法定代表人、董事、监事)之间存在近亲属、相互占股等关联的,也不得同时参加同一 包的采购活动。近亲属指夫妻、直系亲属、三代以内旁系血亲或者近姻亲关系。
8.法律、行政法规规定的其他条件。
七、询价文件获取
此项目公示时间为公示发表日期起的3个工作日,有意向参加的供应商报名时间为项目公示发布日期起的5个工作日(北京时间,节假日除外),并按以下步骤领取询价文件模板:
(1)电话短信方式通知报名联系人;
(2)以“某某单位参加“高速光电探测器及射频信号模块”为邮件标题,将以下内容发送至采购联系人邮箱(民网邮箱号:kdoptics@163.com):
1)投标人全称、项目名称、联系人姓名、电话、电子邮箱;
2)企事业单位提供营业执照pdf扫描件和法定代表人授权书pdf扫描件(含法定代表人和被授权人身份证复印件);
3)军队单位提供法定代表人授权书pdf扫描件、装备承制资格证书和质量体系证书pdf扫描件。
采购人将以电子邮件的形式向供应商发送信件文件。
