2）提供所投YAG皮秒激光治疗仪设备原厂DATASHEET。

3）如果投标人所投的货物不是投标人自己制造的，投标人应得到YAG皮秒激光治疗仪制造商同意其在本次投标中提供该货物的正式授权书（参见第一分册格式IV-9-4）和制造商资格声明（参见第一分册格式IV-9-2）；其他投标产品在中标后具有供货能力。

Qualifications or performance of the Bidder:

1) Provide the Medical Device Registration Certificate issued by the national Food and Drug Regulatory department of the invested equipment.

2) Provide the original factory DATASHEET of the invested YAG picosecond laser therapeutic instrument equipment.

3) If the goods submitted by the bidder are not manufactured by the bidder himself, the bidder shall obtain the consent of the manufacturer of YAG to provide the formal authorization of the goods (see first sub-volume format IV-9-4) and the manufacturer"s qualification statement (see first sub-volume format IV-9-2); the other bid products shall be capable of supply after winning the bid.
