一、工程名称：塘栖镇南苑社区居家养老照料中心装修工程
二、招标范围：工程概算35.3408万元，工期45日历天，施工图纸范围内的装修工程（具体详见招标文件）。
三、报名资格要求：
被列入2016年余杭区小额备选库名单，具备建筑装修装饰工程专业承包（新）贰级及以上或建筑装饰装修设计与施工一体化叁级及以上并具有足够资产及能力来履行合同的施工企业（诚信制裁企业不得参加报名）均可报名参加投标。
建造师资质：建筑专业贰级及以上。
四、报名应提交以下资料：
（1）营业执照原件及复印件、资质证书、组织机构代码证原件及复印件（原件及加盖公章复印件，验原件留复印件）；（2）介绍信原件；（3）项目承包报名书原件；（4）建筑专业贰级及以上证书原件及复印件（原件及加盖公章复印件，验原件留复印件）；（5）拟派建造师未在其它建设工程项目上担任施工单位项目负责人的承诺书；（6）报名时交易员必须到场，需现场出示身份证及交易员证，否则将不予以受理。
五、报名时间、地点：
1、投标报名时间：2016年12月15日至2016年12月19日，上午9：00-11：30、下午13：30-16：00，（节假日除外）逾期不予受理；
2、报名地点：杭州市余杭区塘栖镇公共资源交易中心2楼会议室（塘栖镇人民路183号塘栖镇行政审批服务中心内）。
六、投标保证金：在领取招标文件时、投标单位需提交投标保证金，投标保证金伍仟元人民币整，户名：杭州余杭塘栖城建发展有限公司，开户银行：浙江杭州余杭农村商业银行塘栖支行，帐号：***************。已缴纳区住建局本年度保证金或塘栖镇建设工程投标保证金的投标单位无需再次缴纳。
