资格要求1.中国境内供应商：营业执照要求：投标人具有合法有效的企业法人营业执照、税务登记证及组织机构代码证或证照合一的营业执照，投标时需提供原件扫描件（原件备查）。投标人为事业单位的，应具有合法有效的事业单位法人证书，投标时需提供原件扫描件（原件备查）。投标人为分公司的，提供具有合法有效的营业执照和上级法人单位授权书（授权该分公司投标），认可该分公司和上级法人单位的资质、资格和业绩，不认可同一上级法人单位的其它分公司的资质、资格和业绩，投标时需提供原件扫描件（原件备查）。分公司与上级法人单位只可一家参与投标，同时参与投标的，投标均无效。如出现不同申请人的负责人为同一人或存在控股、管理关系的情况，相关投标均无效，招标人随时有权做出拒绝投标、取消投标资格、取消授标、不签订合同或在合同签订后终止合同等决定，相关申请人须无条件接受，且由此造成的任何损失由对应申请人承担。2.投标人具有有效的GB/T
*****（ISO9001）质量管理体系认证证或者API Q1 质量体系认证证书，并可在中国国家认证认可监督管理委员会网站(http://www.cnca.gov.cn/)核实。如果有国家相关部门发布的最新体系标准，以最新体系标准为准。投标时需提供原件扫描件（原件备查）。注：如所投货物的制造商提供的质量管理体系认证证书是国外第三方认证机构颁发的，则网站查询不适用。3.投标人应为本次投标产品的制造商。本次招标不接受代理商、贸易商投标。本次接受所投货物的制造商全资的销售公司或控股的销售公司，以及接受所投货物制造商的母公司参与投标，此类公司视同为制造商，投标时需提供相应的证明文件。同一制造商仅允许一家投标人参与投标，否则相关的所有投标将被否决。Qualification Requirements:1. Note: If the Quality Management System Certification Certificate provided by the manufacturer of the goods to be bid is issued by a foreign third-party certification body, the website query shall not be applicable.3.The bidder shall be the manufacturer of the bid product. This tender does not accept agents, traders bid. The wholly-owned or majority-owned sales company of the manufacturer of the goods accepted this time, and the parent company of the manufacturer of the goods accepted to participate in the bidding, such companies shall be regarded as manufacturers, and the corresponding supporting documents shall be provided when bidding. Only one bidder fr0m the same manufacturer is allowed to participate in the bid, otherwise all relevant bids will be rejected.
