3. 投标人资格要求QualificationRequirements For Bidder

购买招标文件时，需提供法定代表人授权委托书原件、法定代表人授权代理人居民身份证原件，同时提供上述文件加盖公章的复印件一套。

When purchasing the bidding documents, the bidder shallpresent the original of the power of attorney of its legal representative, theoriginal of the identity cards of the authorized person, meanwhile, submit onecopy of every document above bearing its official seal.

提供各种资质证书的复印件：投标人须提供有效的企业法人营业执照副本（或事业单位法人证书）、税务登记证、组织机构代码证复印件（若营业执照更换为统一社会信用代码，则不需要提供组织机构代码证、税务登记证），ISO9001质量管理体系认证证书。

如投标人为代理商，还须提供制造商有效的企业法人营业执照副本（或事业单位法人证书）、税务登记证、组织机构代码证复印件（若营业执照更换为统一社会信用代码，则不需要提供组织机构代码证、税务登记证），ISO9001质量管理体系认证证书。

The bidder shall provide photocopies of valid duplicateof business license of enterprise legal person (legal person certificate ofpublic institution), tax registration certificate, organization codecertificate (provided that the business license is changed to the uniformsocial credit code, organization code certificate and tax registrationcertificate shall not be required), and ISO9001 quality management systemauthentication certificate.

If the bidder is an agent, it must also provide themanufacturer’s valid duplicate of business license of enterprise legal person(legal person certificate of public institution), tax registration certificate,organization code certificate (provided that the business license is changed tothe uniform social credit code, organization code certificate and taxregistration certificate shall not be required), and ISO9001 quality managementsystem authentication certificate.

针对境内投标人：投标人须提供公司注册地检察机关出具的《检察机关行贿犯罪档案查询结果告知函》；投标人须提供银行基本账户开户许可证；被人民法院列为失信被执行人或被国家安全监管总局列入安全生产不良记录‘黑名单’的潜在投标人不得参加投标，否则其投标无效。

For domestic bidders：the bidder mustprovide the Procuratorial Organ’s Letter of Announcement of Query Result ofBribery and Crime Archives offered by the local procuratorial organ of thedomestic bidder’s registered office; thebidder must provide the account opening permit for its basic bank account; the bidder that is listed as dishonestexecutor by people’s court(s) or in the “blacklist” of work safety demerits byState Administration of Work Safety mustn’tbid . If so, its bid will be rejected.

业绩要求：投标人或制造商近三年（2014年-至今）应具有制造或销售本招标要求的同规格或类似设备（不低于本招标要求）的业绩，并提供近三年的业绩清单，出具相关业绩的销售合同复印件（含合同首尾页、盖章页、设备名称及清单页等）作为证明材料，否则，将否决其投标。

Performance requirements: The bidder or manufacturershall have the performance of manufacturing or selling the equipment the sameas or similar (not below the requirements of the bidding documents) to theequipment to be provided in the past three years as of 2014, provide aperformance list of the past three years, and offer the copy (first and finalpages and seal pages of contact, pages of equipment name and a list ofequipment, etc.) of sales contract(s) related to such performance asevidentiary material. Otherwise, its bid shall be rejected.
