Zhuzhou Qingshuitang Industrial Zone is a key and old industrial base invested and built by the state in the 1960s, with leading industries of lead-zinc smelting and heavy chemical industry. Over 60 years" development of metallurgical and chemical industry has turned Qingshuitang Industrial Zone into a brownfield contaminated by heavy-metals notorious in Hunan Province or even around the entire country. The environmental pollution in the area is endangering the health of residents and the safety of drinking water in the middle and lower reaches of Xiangjiang River. Over the years it has caused a series of livelihood, economic and social issues. Pollution remediation of Qingshuitang Industrial Zone has become an urgent need for improving the local people"s livelihood and eliminating the water pollution problems of Xiangjiang River. In order to combat the long-standing pollution and improve the soil quality of Qingshuitang, improve the water quality of Qingshuitang and the entire Zhuzhou City, as well as renovate the local ecologic environment, the government proposes to undertake the risk-based brownfield remediation of the key industrial zones in Qingshuitang under the support of World Bank loans and advanced international brownfield remediation technologies.

The project will be managed by the Project Management Organization (hereinafter referred to as“PMO”) of the Zhuzhou Municipal, and be implemented by the Zhuzhou Recycling Economic Investment and Development Group Co., Ltd (hereinafter referred to as “ZREIDC”) through the Project Implementation Unit ( hereinafter referred to as “PIU”), PIU will be overall responsible for the implementation of the project as the project client.

This contract is a part of the world bank loan project (hereinafter referred to as "the project") of zhuzhou brownfield remediation project. As the executing agency of the project, zhuzhou circular economy investment and development group co., LTD. (hereinafter referred to as "zhuzhou circular economy group") intends to use part of the loan for project supervision and consulting services. The expected contract term of this contract is two months after the completion and acceptance of all projects, and it is scheduled to start in September 2019. The executive office invites qualified supervision companies to express their interest in providing relevant engineering supervision consulting services. Interested consultants should provide relevant materials to prove that they have the relevant qualifications and relevant experience to be competent for this service, including but not limited to: resume, basic introduction, and description of relevant achievements in recent years. The evaluation principles are as follows :

(1) domestic bidders shall have independent legal person qualification and grade a or above municipal public works supervision qualification;

(2) for similar project supervision performance in recent five years (from January 1, 2014 to the date of contract signing), performance certification materials, such as copies of contract signing, should be provided.

The attention of interested Consultants is drawn to paragraph 1.9 of the World Bank’s Guidelines: Selection and Employment of Consultants [under IBRD Loans and IDA Credits & Grants] by World Bank Borrowers, January 2011 and Revised July 2014 (“Consultant Guidelines”), setting forth the World Bank’s policy on conflict of interest.

Consultants may associate with each other in the form of a joint venture or of a sub-consultancy agreement to complement their respective areas of expertise.

A Consultant will be selected in accordance with the QBS method set out in the Consultant Guidelines.

I. Project Background

Qingshuitang Industrial Zone in Zhuzhou City is an old industrial zone base invested and built by the state in the 1960s. The leading industries are lead and zinc smelting and heavy chemical industry. The development history of metallurgical industry for more than 60 years has made Qingshuitang Industrial Zone a famous heavy metal contaminated area in Hunan Province and even in the whole country. Local environmental pollution endangers the health of residents and the safety of drinking water in the middle and lower reaches of Xiang River. Over the years, it has caused a series of problems in people"s livelihood and economic and social development. The pollution control in Qingshuitang Industrial Zone has become an urgent need of improving the local livelihood and eliminating the hidden danger of Xiang River water pollution. In order to thoroughly control the pollution left over from Qingshuitang, effectively improve the regional soil environment, build up a better water environment of Qingshuitang and even the whole Zhuzhou city, and restore the ecological environment within the region, the government will implement Zhuzhou Qingshuitang Brownfield Remediation Project in Zhuzhou Qingshuitang region by using World Bank-financed loans and drawing on international technologies.

The scope of the implementation of World Bank--financed Qingshuitang Brownfield Remediation Project is within Qingshuitang Industrial Zone, Shifeng District, Zhuzhou City, Hunan Province, with Xiangtian Road in the east and Beijing-Guangzhou railway in the west. From the west to the east, the northern boundary covers the planned Fahua Avenue, the planned Huanbao Avenue, the current Qingshui Road, the current Qingxia Road, the current Tongxia Road, the current Zhuzhou Smelter Group and the Exterior Wall Section of CNSG Hunan Zhuzhou Chemical Industry Group Co., the planned Huaping Road, the planned Zhicheng Road and the planned Fahua Avenue. The southern side is adjacent to Xiang River. The total area is 8.48 km2.

The estimated total investment of the project is about 245 million US dollars (including reserve funds and interest), of which the World Bank loan amount is 150 million US dollars. This project mainly includes the following four parts:

Part I-Remediation of Contaminated Plots: Cleanup and Remediation Activities for Contaminated Plots and Scattered Bare Plots in Residential Areas, including: (a) site preparation; (b) material transportation (including necessary transportation road construction); (c) soil and sediment treatment; (d) remediation of contaminated soil plots, ponds and canals; (e) clean-up of industrial waste residues; and (f) soil replacement works in bare soil areas in residential areas.

Part II-Relevant Remediation and Treatment Works: Construction and Upgrading of Relevant Remediation and Treatment Facilities Required for Pollution Control, including (in particular): (a) Dewatering site; (b) stabilization and solidification equipment; (c) wastewater treatment facilities; (d) construction of industrial solid waste landfill sites in the project area.

Part III-Capacity Building and Knowledge Management: This part will provide Shifeng District of Zhuzhou City with the following support: the implementation of research topics related to the project and capacity building activities, public consultation, brownfield restoration management and development planning, compliance building of environmental quality and industrial pollution, including the establishment of environmental protection information and demonstration centers.

Part IV-Project Management, Monitoring and Evaluation: This part supports the Project Office and the Executive Office to strengthen their management capabilities through training, research visits and external consulting services, including: (a) project management capacity building; (b) project management; (c) external environmental and social monitoring.

II. Content and Scope of Construction Supervision Work

1. Scope of supervision work:

Supervision scope of the Project includes: Supervision service in the construction preparation stage, construction stage as well as handover acceptance and warranty period within the bidding scope (see requirements in the Mission Statement provided by the tenderee for specific work scope and boundary).

2. Content of supervision work

(1) Develop supervision plan immediately after the project design document is received and report it to the client 7 days before the first site meeting; draw up detailed rules for supervision implementation according to relevant regulations and supervision requirements;

(2) Familiarize with the project design document and attend drawing reviewing and design delivery meeting presided over by the client;

(3) Attend the first site meeting presided over by the client; preside over regular supervision meeting and chair or participate in special session according to engineering requirement;

(4) Review the construction organization design submitted by the contractor, the conformity of the technical quality and safety measures and special construction scheme contained therein with the mandatory standard of project construction in particular;

(5) Check the project quality, management system for safe production and the qualification of the organization and the personnel of the contractor;

(6) Check the staffing of full-time safe production management;

(7) Review the construction schedule submitted by the contractor and its adjustment on the construction schedule.

(8) Examine the laboratory of the contractor;

(9) Check the qualification of the subcontractor;

(10) Verify the effect of the alignment measurement for construction by the contractor;

(11) Check the work commencement conditions and sign commencement order for qualified works;

(12) Review the validity and conformity of supporting documents provided by the contractor for the quality of the materials, components and equipment of the Project and make spot check for materials intended for the project by parallel acceptance testing or evidential sampling as stipulated;

(13) Review the application of project fund payment made by the contractor; sign or issue certificates for project fund payment; and report to the client for review and approval;

(14) Ask the contractor to make rectification for works with quality problems and construction safety hazards found in inspection, aside supervision and examination and report such problems and hazards to the client;

(15) Issue work suspension order and resumption order after obtaining the consent of the client;

(16) Review demonstration materials adopting new materials, new process, new technology and new equipment provided by the contractor and relevant acceptance standards;

(17) Accept concealed works, partitioned projects and sub-brisional projects;

(13) Review the engineering change application by the contractor and act as the coordinator of construction progress adjustment, cost claim, contract dispute and other matters.

(19) Review the completion acceptance application made by the contractor and prepare project quality evaluation report;

(20) Take part in the completion acceptance and sign the completion acceptance opinion;

(21) Review the completion settlement application submitted by the contractor and report it to the client;

(22) Prepare and manage the construction supervision documents to be placed on file and report them to the client.

III. Qualification Requirements of the Supervision Company and Staffing of Supervision Engineer

1. Qualification requirements of supervision company:

(1) For domestic bidders must be an independent legal person and have a certificate of construction supervision for municipal public works of A and above;

(2) It must have made achievements in similar supervision works in the past 5 years (from January 1, 2014 to present (contract signing date)); a copy of the achievement supporting documents should be provided, such as a copy of the signed contract.

3. Responsibilities of the supervision engineer:

Responsibilities of the chief supervision engineer:

(1) Prepare supervision plan, formulate work system of the supervision institution and approve the detailed rules for supervision implementation.

(2) Divide responsibility among different departments of the supervision institution and define the responsibilities and authorities of supervisors at all levels; coordinate the work of the supervision institution; evaluate the work of the supervisors in the supervision institution and replace those incompetent for their jobs; make staff adjustment in a timely manner according to the progress of project construction.

(3) Sign or authorize the signing of documents of the supervision institution.

(4) Approve subcontracting project and subcontractor proposed by the contractor and report to the employer for its approval.

(5) Approve the application for commencement of works, construction organization design, construction schedule, and capital flow plan submitted by the contractor.

(6) Approve the special construction scheme and disaster emergency plan submitted by the contractor as per relevant safety regulations and contract requirements.

(7) Approve the orderly construction organization and measures proposed by the contractor.

(8) Preside over or authorize the supervision engineer to preside over the design delivery meeting; organize the reviewing and signing of construction drawings.

(9) Chair the first site supervision meeting; preside over or authorize the supervision engineer to preside over regular and special supervision meetings.

(10) Sign important supervision documents such as commencement notice of contract works, construction suspension instruction and construction resumption notice.

(11) Approve completed quantities and payment application and sign all types of payment certificates.

(12) Deal with change, claim, default and other matters and sign relevant documents.

(13) Make coordination during the implementation of the construction contract and mediate the contract dispute.

(14) Require the contractor to replace constructors or technicians and managers who are incompetent or inappropriate to work for the Project.

(15) Organize the review of the quality assurance system documents, safe production and management mechanism and documents of security measures submitted by the contractor and supervise their implementation; if any hidden safety hazard is found, the contractor should be required to rectify and suspend the construction.

(16) Check and approve the contractor"s construction defects treatment plan, organize the inspection of construction defects and filling of the defect record form; participate in the investigation and handling of engineering quality and safety accidents.

(17) Re-check the construction quality grade of sub-project and unit project, and evaluate the construction quality of the project on behalf of the supervision organization.

(18) Participate in or preside over the acceptance of sub-project on commission by the employer; participate in the acceptance of unit project, completion inspection and acceptance of contract project, intermediate acceptance and completion acceptance.

(19) Organize the preparation, signature and issuance of monthly report, special report and working report on supervision, as well as collection of supervision contract documents and archives.

(20) Organize the review of the project archives submitted by the contractor, and submit special review report.

Responsibilities of the supervision engineer:

(1) Participate in preparation of supervision planning and rules for the implementation.

(2) Pre-view the subcontracting project and the subcontractor proposed by the contractor.

(3) Pre-view the application for commencement of works, construction organization design, general construction schedule, annual construction schedule, special construction schedule and capital flow plan.

(4) Pre-view the special construction scheme and disaster emergency plan submitted by the contractor as per relevant safety regulations and contract requirements.

(5) Review the construction drawings as per the arrangement of the general supervision engineer.

(6) Review and approve the commencement application report, construction procedures and construction defects treatment plan for sub-project or part of it.

(7) Check and approve the construction control network and original terrain surveying plan prepared by the contractor; review the contractor"s construction layout results; check and approve the construction process test plan and special test plan submitted by the contractor, and confirm the testing results.

(8) Assist the assistant engineer in coordinating the working relationship between the participating parties. Deal with the relevant problems occurring at the construction site in accordance with the duties and responsibilities; sign and issue the general supervision instructions and notices.

(9) Check the quality certification documents of the mobilized raw materials and intermediate products submitted by the contractor for inspection; verify their quality; review the engineering construction quality; participate in or organize the delivery and acceptance of engineering facilities.

(10) Inspect and supervise the implementation of construction safety and civilized construction measures at the project site, instruct the contractor to correct the violations; in case the circumstance is serious, report to the chief supervision engineer.

(11) Review the completed statement of quantities.

(12) Check the payment application statement.

(13) Put forward preliminary comments on changes, claims, and handling of quality and safety incidents.

(14) Participate in the quality assessment and acceptance of the project in accordance with the duties and responsibilities.

(15) Collect, summarize, and organize the supervision archives, participate in the preparation of the monthly report on supervision, approve or fill in the supervision log with signature.

(16) Report to the general supervision engineer in time and ask for instructions when major problems or emergency circumstances occur during construction.

(17) Guide and inspect the work of the supervisor, and if necessary, recommend the general supervision engineer to change the supervisor.

(18) Complete other work authorized by the chief supervision engineer.

Responsibilities of the supervisor:

(1) Verify the inspection application form of the mobilized raw materials and intermediate products and inspect their appearance to verify the report of construction measurement results.

(2) Check the use of raw materials, intermediate products and engineering facilities used by the contractor for engineering construction, and fill in the on-site records.

(3) Check and confirm the preparations for construction of unit project (process) by the contractor.

(4) Check and record the implementation process of on-site construction procedures and construction technologies, discover unconventional behaviors and quality hazards in the construction, promptly instruct the contractor to correct, and report to the supervision engineer or chief supervision engineer.

(5) Conduct regular or irregular inspections on the construction site under supervision, and implement side station supervision and follow-up inspection according to the supervision implementation rules.

(6) Assist the supervision engineer to pre-examine the commencement application report, construction procedures and construction defects treatment plan for sub-project or part of it.

(7) Verify the engineering measurement results, check and make a statistics on the day works.

(8) Inspect and supervise the implementation of construction safety and civilized construction measures at the project site, and promptly instruct the contractor to correct the violations in case of any abnormal conditions and report to the supervision engineer or chief supervision engineer.

(9) Check the contractor"s construction log and on-site laboratory records.

(10) Verify the original records related to the quality assessment by the contractor.

(11) Fill in the supervision diary, together with the supervision log after authorized by the chief supervision engineer or the supervision engineer.

IV. Requirements of supervision program

In addition to meeting the monthly qualifications and experience requirements of the company and key supervisor, the bidder must also prepare a targeted supervision program for this project as an important component of the technical proposal selection.

The supervision program should include the following contents:

1. Test instruments and equipment to be invested;

2. Scope and content of supervision

3. Basis and objectives of supervision

4. Supervision organization setting and job responsibilities

5. Supervision procedures, methods and systems

6. Quality, schedule, cost, safety and environmental supervision measures

7. Contract and information management plan

8. Supervision organization and coordination contents and measures

9. Analysis of key points and difficulties in supervision

10. Rational proposals

V. Submitting the achievements report

The bidder should submit the reports such as supervision planning, monthly supervision report and agreed special report. The submission time and number of reports are as follows:

1 Submit the supervision planning prior to the first site meeting of each project;

2 Submit the monthly supervision report for the previous month before the 10th of each month;

3 Submit the supervision completion report within one month after the completion inspection and acceptance of each project;

4 Other reports requested by the owner.

VI. Others

1. This is a time-based contract.In time-based contracts the Consultant provides services on a timed basis according to quality specifications, and Consultant’s remuneration is determined on the basis of the time actually spent by the Consultant in carrying out the Services and is based on (i) agreed upon unit rates for the Consultant’s experts multiplied by the actual time spent by the experts in executing the assignment, and (ii) reimbursable expenses using actual expenses and/or agreed unit prices.

2. The contract is expected to commence in September 2019 and the service period is based on the implementation of the relevant contract package, which is expected to be 24 months. Since the implementation time of the contract is greater than 18 months, the contract will include a price adjustment formula, and the unit rate for the remunerations will be adjusted every 12 months.

3. Equipment provided by the owner includes office buildings, air conditioning, office desks, and filing cabinets.

4. The consulting firm is required to provide a list of self-contained equipment in its technical proposal.

株洲清水塘区域重金属污染环境治理工程世界银行贷款项目的实施范围在湖南省株洲市石峰区清水塘工业区内，区域东临响田路，西至京广铁路线，北部边界从西至东依次为规划的法华大道、规划的环保大道、现状的清水路、现状的清霞路、现状的铜霞路、现状的株冶和株化外墙段、规划的化平路、规划的智诚路、规划的法华大道，南侧毗邻湘江，区域总面积为8.48 km2。

项目总投资估算金额约为2.45亿美元（包括预备费和利息），其中世行贷款金额为1.5亿美元。本项目主要包含以下4个部分：

第I部分--污染地块修复：针对污染地块和分散的居民区裸土地块，实施的清理和修复治理活动，包括：(a)场地准备；(b)材料运输（包括必要的运输道路建设）；(c)土壤和底泥处理；(d)污染土壤地块、水塘和渠道的治理修复；(e)工业废渣的清理；以及(f)居民区内裸土区域的换土工程。

第II部分--相关的治理和处理工程：污染治理所需的相关治理和处理设施的建设、升级，包含（尤其是）：(a)脱水场地；(b)稳定化固化处理设施；(c)废水处理设施；(d)项目区内工业固废填埋场的开建设。

第III部分--能力建设和知识管理：本部分将为株洲市、石峰区提供以下支持：项目相关研究课题的实施和能力建设活动，公众参与活动，棕地修复管理和开发规划，环境质量和工业污染的合规性建设，包括环保信息和示范中心的建立。

第IV部分--项目管理、监测和评估：本部分支持项目办和执行办通过培训、研究考察和外聘咨询服务来强化其管理能力，包括：(a)项目管理能力建设；(b)项目管理；(c)外部环境和社会监测。

二、工程监理工作内容和监理范围

1. 监理工作范围：

本项目监理范围包括：在招标范围内的施工准备阶段、施工阶段及交工验收与保修阶段的监理服务工作（具体工作范围及界线以招标人下达的任务书规定为准）。

2.监理工作内容

（1）收到工程设计文件后编制监理规划，并在第一次工地会议7天前报委托人。根据有关规定和监理工作需要，编制监理实施细则；

（2）熟悉工程设计文件，并参加由委托人主持的图纸会审和设计交底会议；

（3）参加由委托人主持的第一次工地会议；主持监理例会并根据工程需要主持或参加专题会议；

（4）审查施工承包人提交的施工组织设计，重点审查其中的质量安全技术措施、专项施工方案与工程建设强制性标准的符合性；

（5）检查施工承包人工程质量、安全生产管理制度及组织机构和人员资格；

（6）检查施工承包人专职安全生产管理人员的配备情况；

（7）审查施工承包人提交的施工进度计划，核查承包人对施工进度计划的调整；

（8）检查施工承包人的试验室；

（9）审核施工分包人资质条件；

（10）查验施工承包人的施工测量放线成果；

（11）审查工程开工条件，对条件具备的签发开工令；

（12）审查施工承包人报送的工程材料、构配件、设备质量证明文件的有效性和符合性，并按规定对用于工程的材料采取平行检验或见证取样方式进行抽检；

（13）审核施工承包人提交的工程款支付申请，签发或出具工程款支付证书，并报委托人审核、批准；

（14）在巡视、旁站和检验过程中，发现工程质量、施工安全存在事故隐患的，要求施工承包人整改并报委托人；

（15）经委托人同意，签发工程暂停令和复工令；

（16）审查施工承包人提交的采用新材料、新工艺、新技术、新设备的论证材料及相关验收标准；

（17）验收隐蔽工程、分部分项工程；

（18）审查施工承包人提交的工程变更申请，协调处理施工进度调整、费用索赔、合同争议等事项；

（19）审查施工承包人提交的竣工验收申请，编写工程质量评估报告；

（20）参加工程竣工验收，签署竣工验收意见；

（21）审查施工承包人提交的竣工结算申请并报委托人；

（22）编制、整理工程监理归档文件并报委托人。

三、监理公司资质要求及监理工程师配置

1. 监理公司资质要求：

（1）国内投标人具备独立法人资格，具备市政公用工程监理甲级及以上资质；

（2）近五年内（2014年1月1日至今（合同签约时间））有同类工程监理业绩，应提供业绩证明材料，如签约合同的复印件。

3. 监理工程师工作职责：

总监理工程师职责：

（1）主持编制监理规划，制定监理机构工作制度，审批监理实施细则。

（2）确定监理机构各部门职责分工及各级监理人员职责权限，协调监理机构内部工作；负责监理机构中监理人员的工作考核，调换不称职的监理人员；根据工程建设进展情况，及时调整监理人员。

（3）签发或授权签发监理机构的文件。

（4）主持审核承包人提出的分包项目和分包人,报发包人批准。

（5）审批承包人提交的合同工程开工申请、施工组织设计、施工进度计划和资金流计划。

（6）审批承包人按有关安全规定和合同要求提交的专项施工方案和灾害应急预案。

（7）审核承包人提交的文明施工组织机构和措施。

（8）主持或授权监理工程师主持设计交底；组织核查并签发施工图纸。

（9）主持第一次监理工地会议，主持或授权监理工程师主持监理例会和监理专题会议。

（10）签发合同工程开工通知、暂停施工指示和复工通知等重要监理文件。

（11）组织审核已完成工程量和付款申请，签发各类付款证书。

（12）主持处理变更、索赔和违约等事宜，签发有关文件。

（13）主持施工合同实施中的协调工作，调解合同争议。

（14）要求承包人撤换不称职或不宜在本工程工作的现场施工人员或技术、管理人员。

（15）组织审核承包人提交的质量保证体系文件、安全生产管理机构和安全措施文件并监督其实施，发现安全隐患及时要求承包人整改过暂停施工。

（16）审批承包人施工质量缺陷处理措施计划，组织施工质量缺陷处理情况的检查和施工质量缺陷备案表的填写；按相关规定参与工程质量及安全事故的调差和处理。

（17）复核分部工程和单位工程的施工质量等级，代表监理机构评定工程项目施工质量。

（18）参加或受发包人委托主持分部工程验收，参加单位工程验收、合同工程完工验收、阶段验收和竣工验收。

（19）组织编写并签发监理月报、监理专题报告和监理工作报告；组织整理监理合同文件和档案资料。

（20）组织审核承包人提交的工程档案资料，并提交审核专题报告。

监理工程师职责：

（1）参与编制监理规划，编制监理实施细则。

（2）预审承包人提出的分包项目和分包人。

（3）预审承包人提交的合同工程开工申请、施工组织设计、施工总进度计划、年施工进度计划、专项施工进度计划、资金流计划。

（4）预审承包人按有关安全规定和合同要求提交的专项施工方案和灾害应急预案 。

（5）根据总监理工程师的安排核查施工图纸。

（6）审批分部工程或分部工程部分工作的开工申请报告、施工措施计划、施工质量缺陷处理措施计划。

（7）审批承包人编制的施工控制网和原始地形的施测方案；复核承包人的施工放样成果；审批承包人提交的施工工艺试验方案、专项检测试验方案，并确认试验成果。

（8）协助助理工程师协调参建各方之间的工作关系。按照职责权限处理施工现场发生的有关问题，签发一般监理指示和通知。

（9）核查承包人报验的进场原材料、中间产品的质量证明文件；核验原材料和中间产品的质量；复核工程施工质量；参与或组织工程设备的交货验收。

（10）检查、监督工程现场的施工安全和文明施工措施的落实情况，指示承包人纠正违规行为；情节严重时，向总监理工程师报告。

（11）复核已完成工程量报表。

（12）核查付款申请报表。

（13）提出变更、索赔及质量和安全事故处理等方面的初步意见。

（14）按照职责权限参与工程的质量评定工作和验收工作。

（15）收集、汇总、整理监理档案资料，参与编写监理月报，核签或填写监理日志。

（16）施工中发生重大问题或遇到紧急情况时，及时向总监理工程师报告、请示。

（17）指导、检查监理员的工作，必要时可向总监理工程师建议调换监理员。

（18）完成总监理工程师授权的其他工作。

监理员职责：

（1）核实进场原材料和中间产品报验单并进行外观检查，核实施工测量成果报告。

（2）检查承包人用于工程建设的原材料、中间产品和工程设备的使用情况，并填写现场记录。

（3）检查、确认承包人单元工程（工序）施工准备情况。

（4）检查并记录现场施工程序、施工工艺等实施过程情况，发现施工不规范行为和质量隐患，及时指示承包人改正，并向监理工程师或总监理工程师报告。

（5）对所监理的施工现场进行定期或不定期的巡视检查，依据监理实施细则实施旁站监理和跟踪检测。

（6）协助监理工程师预审分部工程或分部工程部分工作的开工申请报告、施工措施计划、施工质量缺陷处理措施计划。

（7）核实工程计量结果，检查和统计计日工情况。

（8）检查、监督工程现场的施工安全和文明施工措施的落实情况，发现异常情况及时指示承包人纠正违规行为，并向监理工程师或总监理工程师报告。

（9）检查承包人的施工日志和现场实验室记录。

（10）核实承包人质量评定的相关原始记录。

（11）填写监理日记，依据总监理工程师或监理工程师授权填写监理日志。

四、监理大纲要求

投标人除满足公司和关键监理人月资质和经验要求外，还需针对本项目编写有针对性的监理大纲，作为技术建议书评选的重要组成。

监理大纲应包含如下内容：

1、拟投入的试验检测仪器和设备；

2、监理范围、监理内容

3、监理依据、监理工作目标

4、监理机构设置和岗位职责

5、监理工作程序、方法和制度

6、质量、进度、造价、安全、环保监理措施

7、合同、信息管理方案

8、监理组织协调内容及措施

9、监理工作重点、难点分析

10、合理化建议

五、提交成果报告

投标人应提交的报告种类（包括监理规划、监理月报及约定的专项报告等）、提交时间和报告份数：

每个项目的第一次现场监理会之前提交监理计划；

每月10号之前提交上个月的监理月报；

每个项目完工验收后的一个月内提交监理完工报告；

其他业主要求的报告。

六、其他

1. 此合同是一个基于工作时间的合同，咨询顾问根据工作要求按照时间提供咨询服务，咨询顾问的费用由实际服务所投入的时间确定，即咨询费用为咨询专家的单位时间的费率和咨询服务所投入时间的乘积，可报销费用为实际费用或确定的单价。

2. 此合同预计于2019年9月开始实施，服务时间根据相关合同包实施情况确定，预计为24个月。因为合同实际实施时间大于18个月，合同中应包含价格调整公式，每12个月单价将调整。

3. 业主提供的设备设施包括办公房屋、空调设备、办公桌椅及文件柜等。

4. 咨询公司需在投标的技术建议书中提供自备的设备清单。
