| 投标人应具备的资格或业绩：（1）投标人具有有效的营业执照（境内投标人）/企业经营许可证明文件（境外投标人），需提供相应证明材料复印件； （2）投标人应当符合如下规定：法定代表人为同一个人的两个或两个以上法人不得同时投标；母公司与其全资子公司和/或控股公司不得同时参加投标； （3）国内品牌产品只接受制造商或技术提供方投标；国外品牌产品接受制造商或代理商投标，如为代理商投标，必须提供制造商出具的唯一授权书原件或复印件加盖公章，否则其投标将被否决； （4）业绩要求（满足①或②）： ①非专利商推荐的投标人：近15年内（2004年1月1日至今，以合同签订时间为准），投标人所投牌号的产品（脱砷剂）具有至少3套应用于PP装置且满足设计周期的业绩（需提供相应销售业绩合同及技术协议关键页复印件及为履行该合同时的原料买卖发票或原料供货合同复印件，全部加盖投标人公章并装订在投标文件中）； 注：A、投标人如为制造商，但非所投产品技术提供方，须另提供与技术提供方的产权关系证明复印件或授权证明复印件加盖投标人公章并装订在投标文件中； B、投标人如为代理商，且所投产品的制造商非技术提供方，须另提供制造商与技术提供方的产权关系证明复印件或授权证明复印件，全部加盖投标人公章并装订在投标文件中； ②专利商推荐的投标人：投标人获得本装置工艺包专利商推荐的可直接认定满足业绩要求； （5）投标产品必须满足招标文件要求的技术标准，投标人必须提供质量检测报告，检测报告内容涵盖招标技术要求的关键指标（对于执行国标产品，须提供国家级检测机构出具的检测报告复印件加盖投标人公章；执行国外标准产品，须提供经审查合格的检测报告复印件加盖投标人公章）； （6）投标人必须接受招标人实地考核，如发现投标文件提供材料与实际情况不符合，招标人有权取消中标资格（需提供承诺书加盖投标人公章装订在投标文件中）； （7）投标人在中国境内近三年没有不良业绩记录或严重质量问题，并出具相应情况说明； （8）投标人应当提供在开标日前3个月内由其开立基本账户的银行开具的银行资信证明的原件或复印件； （9）本项目不接受联合体投标。 |
