5.主要资格

①投标人提供有效的营业执照（营业登记）/经营许可证明文件。

②投标人提供近三年（2017年至2019年或2016年至2018年）经审计的审计报告或企业财务报表（资产负债表、现金流量表和损益表）。

③境内投标人须提供银行基本账户开户许可证复印件或提供基本存款账户信息。

④境内投标人应无行贿犯罪记录，并提供书面声明（详见格式文件）。

6.业绩要求

投标人所投产品制造商或代理商提供相同口径（或以上）的同类产品应在中华人民共和国境内有2次及以上使用先例，并且应为此招标公告发布之日的前五年内，已经验收合格的项目。投标人须提供用户联系方式，并提供项目合同或用户证明作为业绩证明材料。未提供有效业绩证明材料或业绩数量不满足前述要求的，评标价格将在投标价格基础上增加1%。

7.简要技术要求

根据熔石英坯片图纸（图1、图2、图3）要求，完成21件440mm×440mm×50mm尺寸、57件430mm×430mm×10mm尺寸、9件415mm×415mm×40mm尺寸熔石英坯片的加工制造，以及相应的出厂检测，并按买方要求出具检测报告、提供检测数据。产品的最终交付地点为成都，卖方负责包装、运输。卖方须在卖方现场提供符合要求的包装和存储条件。具体详见招标文件。

1.Entrusted by Sichuan Zhonghe Trading Co., Ltd. (Tenderee), SINOCHEM COMMERCE CO., LTD. (Tendering Agent) invites sealed bids by international (competitive) from eligible bidders for the supply of the following goods and related services:

3.Interested bidders may purchase Bidding Documents through website(link:e.sinochemitc.com) from 10th,May, 2020till 15th, May, 2020during the period 09:00~16:00 (Beijing time) upon non-refundable payment of RMB 300.00 or USD42.41for each complete set of bidding documents. Bidders that fail to fill out the purchase record of the Bidding Documents shall be rejected.

4.All Bids must be delivered to the following address before 14:00pm, 1st, Jun, 2020(Beijing time). Time for bid opening ceremony is 14:00pm, 1st, Jun, 2020(Beijing time), the representatives of the bidders are kindly requested to be present at the time.

5.Principal qualification

①The bidder shall provide a valid business license (business registration) / Certificate of business license.

②Bidders will provide audited audit reports or corporate financial statements (balance sheet, cash flow statement and profit and loss statement) for nearly three years (2017 to 2019 or 2016 to 2018) .

③Domestic bidders shall provide a copy of the bank basic account opening license or basic deposit account information.

④Domestic bidders shall have no criminal record of offering bribes and provide a written statement (see the format document for details) .

6.Performance conditions

The manufacturer or agent of the product to be offered by the tenderer shall provide similar products of the same caliber (or above) with two or more use precedents within the territory of the People"s Republic of China and shall, within five years prior to the date of the announcement of this tender, approved projects. Bidders are required to provide the customer contact information and the project contract or user certificate as performance evidence. The bid evaluation price will be increased by 1% over the bid price if no valid performance proof material is provided or the performance quantity does not meet the above requirements.

7.Summary Technical Requirements

21 pieces of 440mm 440mm 50mm size, 57 pieces of 430mm 430mm 10mm size, and 9 pieces of 415mm 415mm 40mm size fused quartz blank were manufactured according to the requirements of the fused quartz blank drawings (Fig. 1, Fig. 2, Fig. 3) , and according to the requirements of the buyer test report, provide test data. The final delivery place of the products is Chengdu, the seller is responsible for packaging, transportation. The seller shall provide the required packing and storage conditions at the seller"s site. See the tender documents for details.
